Winter Wonderland Reservation Form

As the parent/guardian of a child(ren) attending Winter Wonderland hosted by JCHA and Keystone Development, I agree to the following:

I understand that those attending this event will be photographed while participating and that these photographs may be used in promoting this event and future events for JCHA and Keystone Development either in print or on the internet.

I give permission for my child(ren) to be photographed or their images recorded for print or electronic use in promoting JCHA and Keystone Development. I understand it is my responsibility to update this form in the event that I no longer wish to authorize the above uses.
